    Multi-disciplinary assessment

    We meet the GP, district nurse and family together where possible.

    A clinically signed-off plan with named carers, escalation routes and review dates.

    Care begins, gently

    Specialist carers introduced with proper handovers. Plan reviewed at days 7, 30 and 90.
